Medicines Review

The Cambio CDS Medicines Review Application is a software application designed to support the clinician at the point of care in reducing the prevalence of inappropriate polypharmacy, especially in the management of patients with long-term conditions. The CDS Application provides specific information and guidance for the clinician and patient to have a targeted conversation around the pros and cons of the patient’s current medications. Based on this conversation, the patient is then able to make an informed decision about their desire to continue or discontinue one or more active medications. The CDS Application is based on information contained in the Scottish Government Polypharmacy Guidance 2018 document.
