Familial Hypercholesterolemia

The Cambio CDS Familial Hypercholesterolemia (FH) Referral Application supports clinicians with identifying patients that have a high likelihood of FH. Familial hypercholesterolemia is an inherited disorder that significantly increases the risk of aggressive and premature cardiovascular disease, e.g. strokes, heart attacks, and narrowing of the heart valves. If the probability of FH is high according to the Dutch Lipid Clinic Network (DLCN) Score for FH, the application will assist the clinician in writing a referral to a specialist containing all the clinically relevant information needed to make an initial assessment, and will also allow the user to document the assessment in a clinical note. The CDS application uses both structured data available in the local EHR and information provided by the user inside the application to provide an assessment based on the national clinical guidelines.
